It's actually more referred to as Gramercy Park which I would include among the list for sure.

How about Franklin, TN? (outside Nashville). It was definitely upper class from how I remembered it. Also maybe Savannah, GA and Charleston, SC? (although never been to either).
Franklin, TN is still very desirable and a favorite of the country music stars.

Neither Savannah nor Charleston are uniformly posh, but outside their respective historic districts these are the areas I would deem to be so:

Savannah:
Skidaway Island
Isle of Hope
Bluffton (SC)
Ardsley Park

Charleston:
Isle of Palms
Daniel Island
Kiawah Island

Most all of the communities brought up so far have been associated with large metros. I thought I'd mention some that aren't.

Newport, RI
Port Royal, Naples, FL
Biltmore Forest, NC
Pinehurst, NC
Aiken, SC
Sea Island, GA
Petosky, MI
Mount Desert Island, ME
Princeton, NJ
Carmel, CA
Sun Valley, ID
Aspen, CO
Lake Tahoe, CA
Middleburg, VA
Lake Geneva, WI

Generally, the big few are:

California (LA neighborhoods, some SF burbs)
Connecticut ("Gold Coast" and Litchfield Hills)
New York (Manhattan, north shore LI and some of Westchester)
New Jersey (northern areas)
Florida (Palm Beach, Naples, Miami)
