I agree Minnesota is a slam dunk. You may find what you are looking for in New England or NY too within an hour radius of Boston, Toronto or Montreal.

I'll just comment on my area...

If you look somewhere west or southwest of Jacksonville, you can find acreage under 100k in a rural environment with no traffic.
That puts you within an hour of Jacksonville with great health care facilities including Mayo, MD Anderson, and the UF Proton Therapy Institute. There are two decent colleges/universities in the Jacksonville area but you'd also be an hour from Gainesville and the University of Florida. Jax has all the "big-city amenities" you mentioned including an indoor skating rink.
The aforementioned area is an affordable low-crime area.
Property taxes aren't low but they aren't super high and there is homestead exemption. The col is indeed lower and no state income tax.
Has all the following: lake with public swimming and hiking trails OR near a coastal beach...

The only thing lacking is the healthcare marketplace, low humidity and the lack of disaster risk though honestly hurricanes generally speaking have been mild in this region.
