Baltimore is probably having it the worst, the looting which put so much supply out in the streets was a catalyst.

Ohio is bad for heroin, mainly Dayton, Cincinnati and Toledo.

Not a major city, Manchester, NH had an overdoes death rate of 100 per 100k a couple years ago; so maybe among the highest in the country for a place of its size

Southern Ohio, Eastern Kentucky, West Virginia, Upper New England are the worst for opioids right now. Percentage wise Huntington WV (not a major city) has the worst opiate problem in the US.
