I would like to plan a trip to the South that people usually don't want to visit. I want to visit a place to understand what southern poverty looks like. That is why I am deciding to go to Eastern Kentucky. Then I am deciding to go to Nashville to see a true Southern city. Would you consider Nashville to be a true Southern city? If so what major cities in the South have a true Southern feel? I don't think Atlanta or even Birmingham would be choices because they seem too cosmopolitan to have that feel, at least to me.

If you think Birmingham is too cosmopolitan and thus not Southern enough, then there's no such thing as a "true Southern city." Birmingham is arguably the most Southern of the major Southern cities (those with metropolitan areas in excess of 1 million persons).

I think of Nashville as very cosmopolitan because so many musicians have moved there. A more authentic southern city is Savannah, GA, or Charleston SC. Not sure how much poverty there is in Charleston, but there's some in Savannah.

Didn't take long for Charleston or Savannah to be mentioned in a thread where the OP said he/she wants to plan a trip to parts of the South that people usually don't want to visit.

So you think Birmingham is more cosmopolitan than Nashville, interesting...

Nashville is turning into a hipster capital and is also on the rise as one of the most desirable cities to move to. Sure it has "Southern" flare, but Birmingham would be a great place for what you seek, not Nashville

Savannah and Charleston are amazing places that tourists flock to. Doesn't quite fit the bill
