new england, appalachia, cali and colorado are going to be the worst states for drug use

least would be the rural, more conservative states.

the twin cities has a big drug culture as well but i would have thought surrounding areas have a lesser problem

Wrong. Rural areas are typically worse. Especially the “harder” drugs.

The worst states for overdose deaths are rural parts of appalachia and new england which i mentioned.

Personally, I find the areas with the most highly educated immigrants to have the least drug problems.. particularly immigrants from China, Korea, India, etc. Drug use isn't completely absent but it's much lower than the national average in those places at least from my own observation.

Utah along with Eastern Idaho is probably going to have the least underage drinking and drug use thanks to the underlying Mormon influence however there is a problem with prescription drug abuse. Bottom line anyplace that has people is going to have issues with alcohol and/or drugs.
