Parts of Nebraska is very flat (along I-80), but Omaha is quite hilly, especially the farther east you go in the city towards the MO. River. Even though the I-80 corridor is flat, the elevation goes up as you travel west. It was interesting to watch it on the GPS this last trip out. Omaha is about 1200 ft. Ogallala (sp?) is about 3300. So you are traveling imperceptibly UP.

New York is suprisingly hilly and moutainous -although the way down to the NYC suburbs. Same with parts of northern New Jersey.
In the east however West Virginia easily wins the cake for most all around mountainous probably followed by Vermont.
No one mentioned Delaware I think. I found the central and southern parts of the state to be mostly flat. Alot of marshes and beaches along the coast.

Most of Indiana, Illinois, Iowa, Michigan, North Dakota, South Dakota, Oklahoma, Kansas, and Nebraska are pretty flat. No state is perfectly flat, and there are some patches in these states which are hillier, but if it comes to describing flat states, these ones come the closest to fitting the bill. Also, much of Louisiana, Arkansas, and Florida are flat as a pancake, but even they have exceptions.

Let's challenge a few stereotypes:
You might think IA is completely flat, but check out the unglaciated NE part of the state--the locals call it Little Switzerland.
You might think NY is not flat anywhere, but check out the plain along the southern shores of Lakes Ontario and Erie.
You might think Nebraska is completely flat, but check out the NW corner of the state, just south of the Black Hills.
Think of SC as a flat, coastal state? Try the NW corner of the state.
You might think OK is totally flat, but check out the mountainous SE area.
You might think ND is completely flat, but check out the Turtle Mountains on the Canadian border.
You might think CO is completely covered with mountains, but check out the eastern third of the state (are you sure we're not in KS any more?)
Those who say AL is flat have never been anywhere north of Birmingham.
You might think IL is completely flat, but check out the NW corner along the Mississippi River, and Little Egypt in Southern IL (and they call this the prairie state?)
You might think FL is totally flat, but check out the area north of I-10 and west if I-75, and south of the AL & GA state lines.

What exactly constitutes "flat"?
Many parts of these states that are accused of being "flat" are really rolling terrain. In Oklahoma this is the case as only the southwest corner of the state and the panhandle are completely flat. There is an area that is somewhat flat in North central Oklahoma as well. The area along I-44 from Tulsa to Joplin is flat but it is a narrow corridor. Finally the extreme SE corner of the state and part of the Red River valley are flat.
All in all, I would say that probably 15-20% of Oklahoma is "flat". The rest is rolling terrain, undulating terrain, or mountains.
