Employee Allowance and Benefit

for eg, in Singapore, Most big companies offer :
- 12 annual leave
- 13 months bonus (at least one month salary at the end of the year)
- medical claim / insurance

I heard that the employee needs to buy medical insurance and others by himself. is that true?

What are the allowance and benefits for the employee in the US?
