One thing I found odd was that when they got there they waited about some 30 minutes before doing anything. I understand if you had some guy saying he had hostages but he was actively going around shooting the whole time, why not have 10-15 SWAT guys run in, maybe throw a flash bang and mow him down?
Because this wasn't a Tom Clancy video game where the SWAT have a mini-map with a highlighted icon indicating the bad guys. It takes time to gather information and synch up teams, if they just ran in there they'd likely shoot each other.

Around 2:02 a.m. ET: An officer working extra duty in full uniform at the club responds. He and two officers nearby open fire on the shooter, and a gun battle ensues.
The officer working extra duty at the club waits for backup. After three or four officers arrive, officers enter the club and exchange gunfire with the shooter.
The shooter retreats to a bathroom inside the club. No further shots are fired until 5:00 a.m. ET.
Uhh..If they knew the gunman had retreated to the bathroom then why not pursue him inside and take him out? Not as if there would be many places for him to go.

Because if you look at the timeline, 1) he was barricaded inside the bathroom, so they couldn't just "rush in"; 2) he had stopped shooting, so the immediate threat wasn't there; and 3) they had a few hundred people still in the club they were trying to evacuate.
Priorities. Save the hundred you can easily save first, then worry about the few held hostage.
It was dangerous. People were shooting real live bullets at everyone. Better to wait until they run out. Officer safety is paramount.
