Photo ID, signature...I have a hard time figuring out how any of that matters when I can't begin to remember the last time a clerk had my card in their hands.It never leaves mine other than while it's sitting in the card reader.
In fact, whoever has my card in their possession can use it with no problem.

True. I also hate the fact that here in the US waitstaff takes your CC to complete transaction.
Handheld CC scanners, used in European restaurants for the past 10 years, are still novelty here ...
A restaurant is the only remaining place in the retail economy where the card disappears, where you actually hand it over and the transaction is not processed right in front of you. There is an opportunity for it to be copied and skimmed.

I've been to a few restaurants where you pay up front. That is no different than the grocery store.
This includes Denny's, Starvin Arvin's, and every restaurant (about 5) that I've been to in Las Vegas.
