It does seem silly, doesn't it! But he's a GP and his practice is new and border line profitable. The number of patients is still about the same, but payment gets worse and worse. How long can he manage to operate at a loss? You don't even want to think about how much mal-practice insurance premiums are.

Somehow, the old saying about doctors being the last to get paid is becoming true again. Medicare and Medicaid payments often don't pay enough to cover the staff salaries for the time and supplies needed for that patient.

If I were him, I'd join the Peace Corps or Doctors Without Borders, and try to figure out a way to get personal fulfillment in a world where money can no longer be depended upon to be the sole means of that kind of support.

Or bypass the NGOs and just go. When we were young my wife and and I just went to Jordan, unannounced. She had a RN and a year of real experience, and within 24 hours, she was hired for a post a the teaching hospital. After a year, they gave her a real promotion to a responsible position that she could not have hoped for in two decades with a BN in this country. We were there for two years, and never had an unhappy moment.

If I were young and possessed a marketable skill, and things were not going as advertised, I'd be out of here in a minute, rather than put up with the crap that is going on in this country now.

The "economy", and the plight of workers, are now two separate issues. On the one hand, the banks and others who got bailed out of their unproductive debt are in better shape than they deserve to be. Workers on the other hand, are not being helped to the point that they can be a significant contributor to the "saved economy".

The only way this can go is down, the sheer amount of bad debt in the commercial banking world is slowly pulling the entire system down, more bailouts will be in order, until the toxic debt equals the weight of government hubris, when the full value of our monetary system is exhausted, only then will wall street be satisfied with the outcome of their war on workers.

So, how bad is the economy? well, in measurable terms it is bad, simply because the present day debacle isn't happening at the speed of events we watch on TV doesn't mean we can dismiss this debacle as another blip on the US economical radar.
