Check out the brand, "Vari-Kennel". They are a lightweight plastic type of construction but very sturdy. My cockatoo couldn't chew out of one of those. (I took them to vet one time for their microchipping, and plopped him in a generic cat/dog carrier we had handy. Picture this, 2 cockatoos and 2 macaws in back of my Outback. In less than 5 minutes he had chewed OUT of that carrier! yikes)
The dog only needs to be able to turn around, and lie down comfortably. So get the smallest one in which he can accomplish that.
Could he lose 5 lbs safely?

We are gonna start giving her the healthy weight dog food and once it gets warmer here she will be a lot more active. Doesn't the dog need to be able to stand up in the crate?
- I thought my dog was bad at chewing ...... but I guess your birds have her beat!!!

Okay, not to be a pain, but this is the site that really matters when you're talking/thinking about bringing pets: Animal Quarantine Information — Hawaii Department of Agriculture
You must -- MUST -- fill out all the forms, follow all the rules or your poor dog(s) will end up in quarantine. Regardless of how much they weigh, which kennel, which airline they fly.

Do pets sleep in fact? There is no [human] supervisor in cargo. If commercial airlines had a camera there. I do not believe anybody in cabin has a picture what animal do in cargo, what is going on, watch the animals condition. I do not believe that there is access to the cargo part of the plane from the passenger cabin. I wish commercial airlines allowed pets in the back of a passenger cabin, they only allow 3 pets, would be no harm to anybody, pet still be in crates, but under supervision of their owners. Airlines could charge pet owners extra-extra fee.

No pain...
I totally understand that. I have all the paperwork ready to print out. I just need to make sure she can come with us or that we can afford to fly her or the time put into the paperwork and vet visits would be wasted. You know?

And right you are, as I said in my first post to this person. The FIRST thing to do is get all the info from the Dept of Ag.

Oh sure, by "be able to lie down and turn around" I meant from a standing position!

I have another ?
I have read that once the lab in Kansas or whatever gets the bloodwork then you are going into your 120 days. Can you possibly start this too early? Once the 120 days has expired... does it really expire? I guess I am worried if I start too early is there a possibly she won't get direct release?
