Big Island: Volcanoes National Park, Kealakekua Bay.
Kaua'i: Waimea Canyon, Na Pali Coast.
i neglected to add a third thing, because that would be purely based on personal preference. some people love the beach, freshwater, cold weater, i would really leave the third thing open.

I visited the BI, then Kauai. Then came to live on Kauai last summer. I LOVE it here for SO many reasons.
That said, if I were visiting just one, from the mainland (or elsewhere), I'd pick the Big Island. Because seeing an active volcano "up close" (as close as you can safely get) is, for me, a 'bucket list'-type thing. Before I did it, it was a dream; and it actually exceeded my outrageously inflated expectations. (My next one to do is to dive the Great Barrier Reef; I'm going to get certified this year, and hopefully have the $$ for the trip in a few more years.)
There are nice beaches there too, and good snorkeling. And the north part of the island (Kamuela, Hawi, etc.) is beautiful in a totally different way. (Actually, if there's time, I'd go to both, and also see the Na Pali coast, Kokee State Park, and Kauai's north shore - but I'd also skip Oahu entirely, which most people don't want to do.)

Avoid the 'vog'!!
The "vog" on the Big Island is bruuuutal! We were there last year, and while the first week was beautiful, I didn't even leave the air-conditioning during most of the 2nd week. The vog (sulfur dioxide) was a new discovery for us. The air was thick and it really burned our eyes (and our lungs a bit, too). Kauai is beautiful... go there, for sure.

I second your question... especially since one of the boys has asthma and says he feels better here than back on the mainland. Plus, we live in Puna District...

Can't beat the Big Island. We were out there 2 years ago - let's see Volcanoes, Rain Forest, Beach, Waterfalls, Snow on the mountaintop, Markets. We did a 3 day whirlwind of Big Island, I we could have easily spent weeks exploring new things everyday. If you want to stretch out and rest go to Kauai, but I can do that in the mountians of my state - Ga. I could not find many of the things I saw on the Big Island - oh by the way, if you play Golf Great courses on the Big Island, can't go wrong with any! Have fun
