If you as a patient have any doubt or concerns, go find another provider and maybe you can do better. But you should understand that you are viewed differently when you show up with one of these discount plans. If your Cigna plan is like the ones in my area, Cigna pays so low (about 30% of the fees) that nobody is going to chase after you to stay with their office. I find that patients tend to greatly overestimate their value to a practice when they have "insurance."
Many offices collect up front because Cigna is notorious for pre-authorizing a procedure, then denying the claim later. And yes they can do that, and they do it all the time. Then the office has to become a bill collector and collect from the patient. Guess how many patients pay after they already got their surgery? Easier to give a refund than to chase a deadbeat.
