Started with "AARP from Secure Horizons", aka UHC , in CO. Then changed to UHC in CT, then after a typical go-round over a denied claim, dropped them and went with Connecticare in CT.
All are HMOs. Connecticare is a Connecticut-only insurer. For 18 months now all claims have gone smoothly.
However as I get older, am seriously considering dropping the MA and going with a Medigap hi-d Plan F. CT is one of the few states that allow such a switch with no issues. Un fortunately Connecticare does not do any Medigaps.
Yep - usually the smaller MA's are less problematic. Tufts on the East Coast seems to perform fairly well.
I, too, have an hd-F w/Physicians Mutual, but have always felt the premium was too high at $74/mo. Recently learned that Standard Life & Accident out of Texas is offering an hd-F for my age (73) - annual premium $60/mo; otherwise about $63/mo. Not listed on medicare.gov for WI, but are listed in annual State of WI Ins. filings, which is how I found them two months ago. SLAICO should be licensed in CT according to Weiss and financial strength is rated A-.
I sent an email through their website and got an immediate response from an in-house licensed agent. Texas-based companies seem to offer lower rates I've noticed from posters here on CD. You might put them on your list when the time comes.

Only 5 of 14 insurers offer hd-F, from $50 to $88/month.
Medicare's Finder page lists 2 versions of Humana. (?)
Call or email SLAICO directly to be sure. SLAICO was not listed by WI insurance commissioner at all prior to March 2015 - BUT agent said they'd been selling hd-F in WI since Sept. 2011.

Looking at your list, Colonial Penn D-rating (per Weiss); Globe has a B; United American a B-.
I've concluded one has to double and triplecheck everything these days. Not only is Medicare site not current (lists two carriers in WI no longer offering hd-F's in WI for at least two years, and fails to list two others which are [SLAICO; Colonial Penn] - probably more out there I don't know about), insurance commissioner pdfs on filings are not current (see above), and independent agents may not tell you about this, either. Saw a supposed Medicare "specialist" independent agent (60 y/o, in business a long time) about a month ago - he knew nothing about hd-Fs (only sold regular Medigaps, apparently) - asked me to explain how it 'worked.'
