In most cases, a move to another state requires a new plan.
If you are moving from NJ to PA or NY or another nearby state and plan to continue with your current providers, as suggested above, BCBS may require that you switch to a multi-state plan.
If you are moving far from NJ and will not be continuing with the same providers, you need an entirely new plan for your current state of residence. This, of course, means a whole new set of in-network providers.
Your post had "Fed" in the title. Are you a current or retired federal employee? If so, and you're on the BCBS PPO Nationwide plan, you do not need to change policies if you move.

I'm a retired fed as well. And I've had the same BCBS policy (Standard) for 42 years ... 34 as an employee and 8 as a retiree. I lived in DC, Maryland, and Virginia as an employee; Virginia and now Maine as a retiree. All I needed to do was give BCBS my new address whenever I moved.
