Should I be Concerned that Somebody stole my identity? (medical, doctor, take)

Twice now I have received an EOB for services that I did not receive from a doctor I have never heard of.
The first time when I called insurance company I was told it was a mistake. I have a very common name, but it still bothered me how insurance company could make a mistake.
Now I received another mistaken EOB. I won't be able to call the insurance company until Monday. It really has me concerned why this has happened again.
If you have a very common name, it's more likely a mistake rather than someone stealing your identity. Not much you can do before Monday. Do you use a middle name? Maybe you can make sure that's added to your medical records and insurance info so you are less likely to get mixed up with people with the same name.
Seems like they would match id number and address with a name. I'm questioning if it was a mistake? Just don't know.
