I work for an insurance company. Phone calls from providers are always logged and many dental providers actually submit written dental pre treatment requests to determine what benefits are before services are rendered. We respond to those written requests in writing.

Calls are logged but those logs are often unavailable. Similar to how claims are often "lost." Suspiciously, the answer always benefits the insurance company.

Ours are not lost...lSome companies just won't release them without a subpoena...lol
