Why does this not surprise me any?
I like how some of these insurance companies like to dictate to the dr's that so and so doesn't need this treatment, as if they're the ones to know

At one time I was considering going to the Cancer Treatment Center, in Phili , because it was closer to where I lived and I had family in the vicinity. Between the CTC and insurance, would not pay unless I went to the one in Chicago, IL - some 500 miles further out of the way. Makes sense to me

That's not to say that there aren't some devious dr's out there trying to bilk insurance companies either. It happens.
