Has anyone had the lap band ? (insurance, procedure, time)

I go in for evaluation for the lap band and my insurance covers, and I'm sure I will get the answer once I go in the office but wanted to know ahead of time . O kay I plan to get procedure done but I am moving out of state and my insurance will end and if my insurance covers and you have to go back in each month or however that is done for a fill how would that work. I hope it doesn't confuse anyone .
