I have the setup shown in the image above. I've found the instructions for it here:
http://13kwbe213cjc2d2um30c7za9.wpen...codedodger.pdf
I have attempted this a number of times and it is not working. I can not get it to go into the programming mode. The light flashes when I open the cover, but when I do the part where you press 3, 5, 7, PROG the light does not start blinking like it's supposed to.
Does anyone have experience with these pieces of junk?

Have you pulled the cover all the way up til it clicks in place then start the programming?

I don’t know if this unit is battery powered or not, if it is ~ did you change the battery? Sometimes a low or dead battery will cause problems…….

When you press 3,5,7 and program together the light will blink if you have a fresh battery and the slider is fully in the up position, but if you don't know you PIN # then you wont be able to program it.
If you think that you have all of the above correct then read the below site.

If you have tried all the above and still fail then get a ladder and on your overhead garage unit press the red reset button and hold it for about 6 secs. The try re-coding the opener again.
If that doesn't work and nothing else works then contact a garage door company for assistance.

There is a tab on the cover that pushes the top left hand button in when the cover is open. The cover MUST be on the keypad and the tab must be directly over the button so that it can constantly depress the button. Once i figured that out, the programming worked perfectly.
