It is true that PIB will build your plan and you can get a truly custom home.

The main difference between P.I.B. and many of the other truly custom builders in the area is that you will be working with a corporation and an associate or superindtendent. Most of the other custom builders in the area you are working with the builder himself. The builder, who owns the company, will be at your job site every day.

THAT is the main difference, and for the price range, you might as well go with the personal touch of one of the many high quality custom builders in the area.

I'm not sure what you mean by "truly custom" when in both cases the builder will provide the exact home you want, assuming you're willing to pay for it

I also have friends/colleagues who have gone the other route you mentioned where you work directly with the builder. That has its pluses and minuses as well. One of the big advantages of the 'corporate' custom builder is that they usually are more organized, have their process down, and fewer things fall through the cracks. The big minus, as you mentioned, is that you aren't working directly with the builder, and the 'personal' custom builders do seem to stand behind their product a little better from what I have seen.

Again, this is just based on my personal experience and hearing from others who have built custom homes.
