Anyone know the health department in houston's mailing address?
Ok I have posted my problem/issue in the georgia forum(you can check it their and read it).But anyways anyone know the mailing address of the health department in houston(if so can you post it on here?)And anyone also know the mailing address of the vital record's office in houston?If so please post it on here.The Reason is because I'm trying to mail them my hospital birth ceritificate,so they can then send me a state issued birth certificate.And since I only have one copy of my hospital birth certificate,which place is better for me to mail/send to?Should I send it to the health department or vital record's office?The lady at the united way in atlanta(which is were i reside)told me to send it to the health department.The reason im doing this is so i can get a georgia id and apply for social security(since they seem to not accept hospital birth certififcates.)Anyways I would appriacate some help.
