Default Birth mothers who want to protect privacy a few days left

To all birth mothers who want to protect their privacy: only a few days left for you to opt out. Birth Certificates search will be opened on Nov. 15.

First of all the Bill was quietly passed in 2010.
Nov 15, is the date a search of court sealed records becomes available. It is not as simple as it sounds as the search process starts with the Midwest Adoption Center (MAC), then onto the Confidential Intermediary Services of Illinois (CISI) and includes court.
To become an anonymous parent papers must be filled out and returned to the state (CISI office) befoe NOV 15, 2011. If it not received by NOV 15th, any adult child can request information on his/her original birth certificate. The registry is an extra level of protection for the birth parent. It does not stop a search request. The entire procress may take serveral months.

I have a friend freaking out about this....
How does one obtain the forms to remain anonymous.

I did a quick search thru these websites and couldn't readily find them.

Ok...I took more time and actually found the forms....so anyone looking for them here ya go.
