You can do something similar for your username, but I would say you wouldn't have to go so far. (I would say you wouldn't have to worry about having a different username for every service, but I certainly wouldn't argue with anyone that wants to).

So now, all I have to remember is my phrase "Guitar String" and my rules. Might seem overwhelming at first, but really, once you do it it becomes pretty much second nature.
Except what do you do with systems that force you to use a different password every time you reset it? Or when it expires?

I have yet to find a service that imposes either of these two inconveniences (password expiration or unique-to-user passwords). The only time I ever ran into this was on a Windows Server that I had to remote into occasionally over a period of a few years to grab files (that was always awesome).

Really? I mean, really??!!

I have to change a password for one system or another several times a week. You must not deal much with entities that are required to maintain a high level of security on their systems.

Any Government system from USPS.gov to job searching boards for your local municipality does this. Yahoo just started something similar with "your password is too similar to your old password." I've run across it on several other sites too, and of course at work. Paypal does it, CCBill, and a lot of financial institutions have started doing it.

I live by KeePassX these days for this reason. I simply can not keep all my passwords in synch, especially for sites I may only use once a month or even once a year.

I use Robohelp 8 to make a multi tab / multi page password Html Help file. Been using it a long time but lately I can't find my master Project file... It somehow got lost among my crashed OS and broken hard drives. I might have to reproduce it along with my temp updates listed on NotePad.
