Who is getting hit with the 4% Medicare surtax? (bonds, stocks, real estate)

I am in the same boat as you... I am revising my strategy and don't know of many options... I have real estate rentals and stocks but they are going to be taxed higher... I am thinking about moving them to tax-exempt municipal bonds, any recommendations on where I should look? Stocks are too volatile and with the additional tax, I think I will go somewhere else...
