Now could I use a resume like this to apply for a "Customer Service Representative" position? Would the "Project Manager" title send any mixed signals?

I don't understand why the "project manager" paragraph is at where it is at. Does it represent your current position? There is nothing to transitions the reader from your personal identifiers to "project manager."

My resume is titled "Project Manager" because that represents the experience I have. I have 5 years experience as a Project Manager. And now I'm applying for a Customer Service Representative position.
So should I change the "Project Manager" title to "Customer Service Representative"? I have experience as a Customer Service Rep, but it's very minimal.

Probably the most important thing to me is that the applicant wants the job I am offering, and wants to work here, not just get any job. Your title makes it look like a general resume sent out to many employers rather than tailoring it to the position. Reading on, if I do not see specifics related to the job soon after it would be going to the bottom of the pile. I made a hire last week that on paper has less experience than several others but obvious, sincere enthusiasm for working in our group made a big difference.

Your resume shouldn't be titled as anything but "resume." You list your work experience under "work experience."

It doesn't take that long to tailor a resume to a position; you are doing yourself a disservice by shotgunning out mediocre resumes.
