There's a place on Lake Mead and Buffalo I enjoy called Grape Street Cafe. Excellent food and atmosphere.

Settebello in the District. That's as authentic Italian as it gets in LV.

The rest of my favorite Italian restaurants are located in casinos, so I won't mention them.

Grape Street is my favorite too, TomasG3, and it's the best for certain things. But the most authentic, because all of the owners and staff are from Italy, is Gina's Bistro on Durango just south of Flamingo. Best Italian food outside of Naples too. And if Gina, who started it then sold it to employees, stops by, you're in for a treat as she looks like an Italian movie star. They treat you like family there too.

What gives Tomas, you writing a travel book for Vegas, LOL? I do like the "best of" Vegas threads though. As far as Italian, what I am missing the most is Carrabas. I miss their spicy chicken Sicilian soup and Mezzaluna pasta.....But alas, once again my favorite restaurants are missing from here up PNW. My favorites in order of preference are:

Carrabas
Bucca Di Beppos
Battista's Hole in the Wall (iconic)
Casa di Amore (best old school Vegas style Sinatra music nightly)
East Side Marios (they closed down years ago and I used to LOVE their food)

Maggiano's. Classy. Great food, and nice authentic decor. Bootlegger's is fun for me. open 24 hours. They have some red eye specials, and breakfasts, too.
