If we live long enough, many of us will require some sort of ongoing care. So assuming we can afford that care, we now have to worry that our caregiver will somehow turn us in as incompetent. I wonder if having our employee sign a non-disclosure agreement would nullify their ability to do this?

I wonder if there's some way to set up estate/trust/living will/health care proxy paperwork ahead of time, in such a way as this situation would be made impossible?

Yes. A major goal of estate planning is to establish what will happen when/if you lose the ability to take care of your own affairs. I've had a number of clients who believed estate planning was just figuring out who gets what and the best way to make that happen. They don't leave my office thinking that.
