The most rural county based on people per square mile is Garrett County in Western Maryland.

Garrett County, and Western Maryland as a whole, is very rural. The Eastern Shore is also quite rural in many spots. In both places, unlike some other rural patches of Maryland (parts of Frederick, Carroll, Cecil, Charles, etc. counties), I don't see these two areas as being swallowed by sprawl any time in the near future just due to their sheer distance (i.e. beyond a reasonable daily commute) from the hub cities of DC and Baltimore.
