Stay in South Beach, you can walk around South Beach or go to the beach. This way you will be in walking distance to shops, restaurants, grocery stores, liquor stores etc. In the evening there are plenty of things to do in South Beach from Jazz clubs, to dance clubs, or just people watch from any of the hip bars. If you stay in downtown Miami you will be much more limited to what you can do. Then you can sleep in, take a cab ride to the port at noon. This way you will be able to book all your spa appointments or reservations at the restaurants ahead of many other people.

I would love to go on a Cruise, where you are looked after 24/7 and pampered silly. But I am put off by the rep the cruises have old middle aged couples and denture cream.

^^^Then don't go on the ships or trips that have the older clientele. Go to the cruise locations and ships that capture the younger audience. Like the Caribbean or the Bahamas, don't go on a Two Week Canal trip which will have an older audience.
