Link to the preliminary fact sheet on the bridge collapse from the FDOT.

The most important part of that fact sheet IMO:

Under the terms of the procurement issued by FIU and due to the unique characteristics of the design of the bridge, an independent, secondary design check was required.
Under this project, it is the responsibility of FIU’s design build team to select the firm used to conduct the independent, secondary review. The firm selected, Louis Berger, was not FDOT pre-qualified for this service, which is required under FIU’s agreement with the state. FIU’s design build team is responsible for selecting a pre-qualified firm and ensuring this process is followed.

I must say, you are doing some great work here, arekusu. From what you've uncovered, it looks as if the design-build team messed up in their selection of the review company. It's possible that a waiver was granted in order to use the company, though.

Looks like a beautiful (but expensive) walkway. It's amazing what gets built when the locals don't have to pay for it.

What happened will be scrutinized in every aspect.
Laws, Regulations and Inspections will change so this will never happen again.
Just like what happened after the Parkland school shooting.

Now is the time to talk about causation and prevention... tht is the problem, after any tragedy people say now is not the time... well when it finally becomes the time nobody remembers or cares anymore and history repeats itself over and over and over. We are adults, we are not delicate fragile children, even in the worst times you have to swallow emotions and see what could have been done differently and what can we do now.

I'm not a structural engineer but does anyone actually know what a stress test is? Or if this is commonly done on bridges over open roads?

Those might be 2 helpful questions to ask before we place to much blame on the people doing the test itself (rather than the design of the road)

It's so funny to hear news reports that people are trying to figure out what went wrong. Isn't it obvious that the cables were not decorative, and that without them the bridge was not supported and that is obviously what went wrong? ... or is there something I'm missing?
