Yup, some things in the Navy never change ( and that is good )

When you are " the skipper" you will be relieved if something happens to that ship.

One of the things I always admired when I was in the Navy was the way they put responsibility on you. If it was on your watch when they went fubar then it was your responsibility "Not on my watch". The best thing was Officers, Chiefs, 1st classes, down the line were held to the same standards as an e-1.

I was on the USS Esienhower, just pulling in after a med cruise and the wind pushed the carrier into a tanker about 500 yards from the pier! We were that close and the CO ended up losing his job for it. You could see into the CO stateroom from the pier afterwards.

He was not even on the bridge when it happened but he was still responsible!

Admiral Kimmel knew what was coming:

In The World at War a naval serviceman, who had been situated alongside Admiral Kimmel during the (Pearl Harbor) attack, recalled that as Kimmel watched the destruction of the fleet, he tore off his four star epaulets and replaced them with those of a Rear Admiral, in apparent recognition of the impending end of his command of the Pacific Fleet.

Pretty harsh.

ENS Nimitz ran USS Decatur aground. He received a letter of reprimand. How times have changed.
