Or is it just a spread out suburb with no central area for shopping etc.

I live in Wauwatosa and don't know of any kind of downtown area in Brookfield -- but I could be wrong.

There's a lot going on down the busy Bluemound road including Brookfield Square; plenty of shops; restaurants; etc., if that qualifies as a downtown

No that doesn't... it's a retail area but not a Downtown. I would say the 'downtown' of Brookfield would be Elm Grove.

Yeah, I don't think there is a traditional downtown either, there is Bluemound rd with tons of shopping and restaurants available, but it seems more like a heavy, car-centric, retail area. I guess I think of a downtown as a more walkable area. There seem to be pockets of shopping concentrated in different areas in the city, instead of a traditional "downtown".

Well the questioner connected downtown with a "central area for shopping". That would be all the retail stores, businesses, entertainment, restaurants, and even the mall and area around it and going west down Bluemound road. Since we're now getting into what is regarded as the downtown of Brookfield per se, that certainly wouldn't be Elm Grove which is a completely separate suburb. It would be the area near the Brookfield library on N. Calhoun road with strip malls, police station, etc. Really nothing much there but it's what's regarded as the downtown of Brookfield per se. Brookfield really doesn't have a downtown though. Milwaukee and Waukesha are places with a traditional downtown.

Are you saying that Elm Grove has a downtown main street with stores etc.?

Yes, it's not large, but it does have some stores and restaurants. As you said, it's more of a downtown main street than a full fledged downtown, which the Brookfield area (Elm Grove/City of Brookfield/Town of Brookfield/Butler) doesn't have.

Yeah, that is probably the closest thing to a "downtown" Brookfield has, they also do the 4th of July Parade there. I call that area Ruby Isle, though.
