1980 film with Joe Spinell. The dank apartment, Dirty and gritty era NYC streets and allys and the fact he played the worst nutcase ever to be put on screen, With his mumbling, grunting and talking to his mannequins.

Spinelli was great in Maniac, but worst psycho ever onscreen goes to Javier Bardem as Anton Chigurh in No Country For Old Men. Dude's portrayal of an unrelenting, souless hitman was absolutely chilling.

As for movies with a dark, gritty feel to them, I believe the 1986 cult classic Henry: Portrait of a Serial Killer fits the bill. My father watched it once and stated the setting was more depressing than the sordid storyline. Overall, the entire film feels dirty.

Trainspotting sure did its job of discouraging heroin use though. It was a painful movie to watch but I love the "choose life" message at the end.

That movie with the guy that lives in an apartment with a bunch of singing cockroaches depresses the hell out of me.

Insomnia with Al Pacino (haven't seen the original). I'm a horrendously light sleeper and have to have total darkness as well as total silence. His character's inability to sleep filled me with more dread than him trying to catch a murderer!

Agreed; I felt like I needed to scrub my insides with a Brillopad after watching that one.

Another "dirty" feeling to me is Sid and Nancy. Their heroin use set against the grittiness of New Yorks lower east side during the late 70's was captured perfectly. On the same line would be the less-known Al Pacino film Panic in Needle Park, about heroin addicts in New York.

If you want to feel really dirty, check out the documentary on GG Allin.

Cruising (1980): Cruising (1980) - IMDb

Filmed on location including gay bars with actual customers as extras:

Cruising (1980) - Pacino dances - YouTube

Gay activists were not pleased.

Just thought of another one: Saturday Night Fever. If I lived in that area, during that time, and only had those people to associate with, I'd move away as quickly as I could, or jump off that bridge. Soul-suckingly depressing!

Enter the Void. What an awesome movie, but holy hell was it dark, sick, and depressing.

The movie for which Melissa Leo won an Oscar. The setting was Indian reservation in Canada in winter; just cold, barren snow, very ugly & depressing.
