Question Deaths During Filmings

IMDB lists the following movies as having a death during filming. Anyone know what happened in each?

I know that Vic Morrow and two child actors were decapitated in a helicopter accident during the filming of "Twilight Zone."

I also know that Brandon Lee was killed by a blank shot that fractured into fragments, 5 of which pierced his chest and several lacerated his heart.

Plan 9 From Outer Space lost Bela Lugosi. He was to appear in the film but died before production started. Ed Wood, being Ed Wood, had some stock footage of Lugosi he threw in which had nothing to do with the film itself. For the rest of the scenes in which Lugosi was to appear he had his wife's chiropractor fill-in carrying his cape over his face to disguise the fact he looked nothing like Bela Lugosi.

A stunt pilot died in a plane crash during filming of "Top Gun"

British Comedian Marty Feldman died of a heart attack while filming "Yellowbeard."

Jerome Rodale died during an interview on the Dick Cavett Show.

Christine Chubbuck committed suicide on camera during a live broadcast of "Suncoast Digest" in Sarasota, FL in 1974.

According to IMDB, the death during "C'era una Volta il West" was a suicide.

There used to be some series on TV with a guy named John Erik Hexum. I just remember that a long time ago, like in the 80's, this guy held a prop gun to his head and pulled the trigger and there was a blank in the gun and it killed him. I could have all the details wrong, but I think that's what happened.

You're memory serves you well. The show was, Cover Up.

However the 80s weren't exactly, "a long time ago."
