It's a percentage (20% i think, but am not sure) and not much off whatever that percent is, unless it was a complicated case and the attorney makes an application to the court for more.
As to time, it depends on when you stop treating so "permanency" can be assessed by doctors. Assuming it's a run of the mill case, w.comp. cases can settle and be over within a year. But like everything else, they can linger. It depends on the nature of the case, sorry, as I know that is not what you are looking for, but "it depends..."
