Default Unemployment EB form

Just received unemployment yellow EB form.No dollar amount in claim balance.Is this correct or should there me a dollar amount like on Tier 1 and 2

it is correct, fill out the top yellow and mail when said, then call on date or internet for your money due. You will receive another letter telling you the amount and also receive when you first call in a balance. No worries!!!!

How long after you claimed your last Tier II week did you receive the EB info in the mail? Is there a seperate letter you get telling you if you qualify?
