Another option would be to go online and find one of the airport transportation services. They have minivans and SUV's available to transport you to/from the airport. Many of the drivers are also willing to do some private work on the side for extra $$$. Just ask and most will be glad to give you their number and take you where you need.
If you are staying in the Times Square area, you can always arrange a car service through your hotel.
As for the rate, it depends upon traffic and the route chosen. $12 is at the very low end of the rate scale, making every light, and little to no traffic. You could pay $25 for the same trip, depending upon those variables, as Medallion cabs to not have a flat rate. $2.50 upon entry/$0.50 per 1/5 mile or minute, depending upon how fast the cab is moving is the current rate structure. Gratuities are also not included, so I would count on a total base fare closer to $20 with light traffic for a cab, plus peak hour surcharge (4-8pm) or night surcharge (8pm-6am).

I am not 100% sure...i am not sure a yellow cab is willing to put 5 passengers in one van. I know i've tried to put 5...could be 6. I believe the refusal is for insurance issues.
Finding a larger taxi when you want it is easier said than done, especially in Times Square. I've also been in larger taxis without the middle seat. There's a lot of leg room, but no extra seating capacity. I recommend having a Plan B, or at least expect to take two cabs and hope you get lucky.
There are a number of them from my experience but I notice every time we get one they turn off their meter...
One time my coworkers and I were getting off work and we got a taxi that was willing to take 5 people. The dude turned off his meter and drove us 4 blocks really fast and he then started texting on his phone. I was in the front seat and I said, "hey stop!" he looked at me and said stop what? and BAM we hit another taxi. We got off and didn't pay, the dude was basically in a verbal fight with the other taxi driver. I don't think he noticed or care we left.
