Good evening, I'm posting today because I believe this community can have some valid input locally.
My grandmother recently suffered a stroke and will eventually be moved to rehabilitation facility for post stroke therapy. She has dementia and her stroke recently crippled the entire left side of her body. She has medicaid and some type of supplemental insurance.
Has anyone here ever had to deal with a similar situation with a loved one?
I am looking for suggestions based of personal experiences with rehabilitation homes and services in the NYC area. We are located in Queens.
Usually the hospital where she was treated assigns a social worker who will make a recommendation. The place that I used was terrible and is now closed so I regret that I cannot be of much help.

Is she assigned to a social worker? Make sure there is someone diligent on the case so that you get all of these things together. They are a big help.
In this case we are having some serious issues with the hospital, so I don't necessarily want to wait for their recommendation Everything has been so slow motion with these guys and we are trying to have a proactive approach.
I know people who have done well at New York Presbyterian and Mount Sinai with respect to rehabilitation programs. For physical rehab, I also know of great results with NYU as well, and they do also provide stroke rehabilitation services, just that the people I know have not had any cognitive therapies. If you do not mind a drive to White Plains, Burke is also a very good facility to consider.
With any facility that is recommended, or that you come across in research, make an appointment to visit the facility, and then push for your favored choice. Sometimes, it comes down to beds in the acute rehab facility, so you may not always get your preferred choice, but if there is a facility that you will not accept from a social worker, be vocal about it.
