This is a little odd, but I thought I'd give it a try.

I've been stubbornly looking for a kind of blanket, a thermal acrylic blanket, that seems to no longer exist. While I've been dragging out the search, I have an existing blanket has not only become worn, but the weave has stretched out so that, there are gaping holes near the satin trim - you know, like from pulling up the blanket. I don't think any charitable organization would want something this shabby, but it kills me to just toss it in the trash, considering that there will still be desperate people this winter in need of anything that might help keep them warm. Are there any alternatives to tossing it?

Wait till the first cold day and just give it to the first homeless person you see. Or find the nearest homeless shelter.

H&M has a little film on their home page and will take any textile, any condition. You bag it and drop it in the box at the store. The film will explain everything.

The thought crossed my mind, but I guess it's embarrassing - usually people give away things that are still in good condition.

Well if it’s too thin to keep someone warm then maybe not. Otherwise they won’t care .
