my unemployment appeal took so long that i gained employment before i won my appeal. my benefits were $350/wk. My new job was supposed to be full time but was not and i have discovered that my new job should provide 120% of my Unemployment Benefits. So i need to reopen my case. Is there a specific form or can i just write a letter to my unemployment office? and do i need to send it to the original employer as well?

You would be far better served to contact your unemployment office directly rather than asking on an Internet forum where you have no idea if you're getting valid advice or not.
