Default Extremely confused about VA health Form requirements-Going from Private K program to public Prince Willaim Cty 1st grade

Hoping someone can clear this us for me as I am very confused. Our child will be attending a private kindergarten this fall and I have been given the VA health forms to be completed. Now since this requires a physical, TB screening (within 30 days of entrance) and confirmation of vaccinations....I will be taking my child to a dr. to fill this out. If my child has all the form and requirements completed for this fall will my child then need ANOTHER physical and TB screen to enter prince william cty public first grade program? Or will Prince William county honor the Va health forms from the year before when my child attended private kindergarten? Extremely confused......

I do not know about Prince William County, but in Fairfax County it is standard procedure to get a VA Health Form on an annual basis. I know that we have not had to have a TB test on an annual basis, but everything else (physical examination, updated vaccination information, etc.) had to be done within the past year. In our case our kids also need the same VA Health Form for summer camps, so it becomes quite useful throughout the year.

Agreeing with OrangeFish.

My kids have been in both private and public. Public will accept the forms used during the previous year as long as the date on the form has not gone past one year. If you turn over your physical form to public school next year and it is within the one-year date that the doctor signed it, you will be fine. If it's past, you will need a new form filled out.

As far as I know, PWC schools do not require a TB test. When you take your child for her well-child visit next year, just have the forms redone.

That's interesting - my kids go to Fairfax Co schools as well but when I asked the school secretary if we would need an updated physical this year, she said, "No, we don't need one after the first year." Hmm . . .

That's strange.

Well, here's the information that is posted on the FCPS web site. Who knows how they use it, but . . .

The CONTINUING STUDENT bundle contains several forms that are commonly used for FCPS students coming back after the summer break. This file is about 1MB, still a relatively large file that may be more than you want to open.

Once my kids were in ONE school system, they did not need to get a physical form done each year. It only was required when they changed school systems or during a year when a certain shot series was due (I think there are some boosters required in 6th grade). And even then, I only had to provide an updated immunization chart.

For my son, he went to private Kindergarten, so they wanted a physical form. In first grade, he went to PWC public schools, so I had to provide an up-to-date physical form. He then stayed in public schools until 4th grade where I never had to provide anything further.

In 5th grade, he went back into private school and I had to fill out another physical form for the new school. He is entering high school this year and I have to provide the high school with a form.

thanks everyone for the info as its been helpful....It apprears I would need to get my child and updated physical for 1st grade. I was given a form for the TB that said it would have to be a negative result within 30 days of entrance.....Have another question. Does the 1 yr physcial date go from when you register the child (I'm not sure when registration for 1st grade is for PWC) or the start date of the school year (ie after Labor Day)?

I always thought that it was done by the date on the form along with the date that the school required you to turn in that form. So, if you registered your child in June and provided the form then it would be fine for them to take (even if the form expires in July and school starts in September). But, if for some reason, you didn't have the form with you and provided it right before school started it would still have to be within the one year date that the doctor signed it.

You do know your child does not have to have a tb test if you have not lived outside the US and he does not have any of the risk factors. I just did these for my kids since we are here for the next 6 months and I had my doctor in NM fill out all of our forms. All he did was fill out the forms and my kids were not at risk for TB, of course I could get there tomorrow to register them and they tell me they need more info. I am hoping not but you never know.

I think the physical form is based on 1 yr from the date on the physical without regards to a new school year. That is my understanding from the Arlington school district. I guess PWC might be different. This is all good for me to know since if we opt to come back to NOVA on a permanent basis we will be living in PWC.
