Are there any nice cities with a reasonable commute to Portsmouth?

Huntington, WV would be probably the nearest "city" of any size, and it's almost 1 hour commute. There are parts of Huntington that are charming, and parts not so charming. You might post your question in the WV forum. I lived in the Charleston, WV area up until 10 years ago but that is too far away. My ex-husband never liked Huntington, so I never got much chance to explore the cultural scene there. There are a lot of problems with drugs, but it's getting so that every "college town" especially the more rural ones are meccas for drug dealers. Lack of extensive police force, high unemployment or menial employment for many locals, lots of money problems, lots of students, all that makes a good opportunity for drug dealers to move in. The old coal camp counties in very southern WV have had pill-mill doctors and an oxycontin racket going for years. But if you chose your neighborhood carefully, I think you could live in Huntington comfortably and Marshall has med school and nursing degrees.

I don't know anything about Ashland or Ironton. I knew people from Chillicothe who'd moved to Charleston for employment, so I don't think there is much going on there, either.
