Does the tone of this email seem to "harsh"? I got called into my supervisor's office after sending out this email. I receive incomplete paperwork all the time. This paperwork was received two weeks after I sent a reminder email on the process for completing forms. And this wasn't the only packet of paperwork that was incomplete. I received two more in the next few days. The directions are on the forms and in our work handbook. I get frustrated because there is always some kind of excuse as to why it is incomplete. My supervisor's response when I complain is that the staff members are busy or that it's been this way since she's worked there (which is 15 years). Need advice on how to handle the situation.

A purchase request for xxx was faxed to xxx at the business office fax number. Since this is a purchase request, it was given to me. There are multiple issues with this purchase request. It does not include the date of the event or who is attending. Since the budget to be charged is xxx, it will need xxx's signature. Permission to attend forms will be needed for each attendee if they have not already been submitted. A transportation request will need to be submitted as well, if you have not done so already. All of this will need to be sent to xxx for approval first since it is travel/training related. After approval, the forms will be routed to the proper department by xxx.

Obviously you harmed someone's self-esteem with your insensitivity. Perhaps you are out of touch with today's culture and fail to realize that insensitivity is is the most abusive and terrible crime that one can commit. I think you need to spend time at some kind of re-education camp to correct your propensity to harm others by hurting their feelings.
