yes, common to do this, but there is most often a 'drop charge', so they can relocate the car back to original departure point. (tho they seldom actually do this). It is good to check around, as some drop fees are quite high to avoid all the cars ending up in one location.

Thus it can be cheaper to drive the opposite direction you previously planned, if the car company has too many cars at your arrival destination, and too few at your deprature point. (you get to relocate their car for them, and you pay for that privilege (there will still be a 'drop charge'...... nice guys!!)

The insurance usually covers you if you're in a car accident with their vehicle or if someone hits you. If you have your own insurance, as a US driver, it covers you the same way without having to pay extra for it when you rent a car. I'm not sure how it works with international drivers. And you don't usually have to purchase their insurance. But if you don't have private insurance coverage while you're here and the rental vehicle is damaged, you'll be liable (meaning, paying) for the damages yourself. If you have insurance, the insurance company pays for the damages.

Something you might want to check into is if your driver's license from your home country is considered valid here in the states. You may not be able to rent a vehicle unless it is.

And the "Gold Rate - Includes 3 additional drivers" would mean that if you have three other legally licensed drivers in the car that they could drive it and would be covered by the insurance, too. If not, you're the only one allowed to drive the vehicle and be covered by the insurance.

And there's really no way for anyone to give you the approximate cost of insurance for a 22 y/o man on the "cheapest" car. Each area and company will figure out their own rates of what is the "cheapest" car. That, and insurance companies closely guard how they come to their rates for each demographic of drivers. Just know this, here in the states, the group with the absolute high insurance rates are men between the ages of 18-25.
