Well, I DEFINITELY live up north, and my boys were raised to call any adult sir or ma'am, and I call waitresses, counter staff and sales people sir or ma'am if I don't know them well enough to call them by name. A hangover from the rearing I got from my parents and 20 years in the Navy.
It's not merely a matter of age, but a minor point of etiquette handed down from a strict upbringing in my case. When dealing with customers, I generally use an honorific with the last name unless I happen to be on really good terms with them, in which case I MAY use their first name. It costs me nothing to be polite (maybe a tenth of a breath to use a simple term indicating respect for the person or position.
