I had a similar thing happen to trying to ship golf clubs to someone in TX via USPS. They were delivered back to me instead.
Turns out, because my label had "To:"on top, and "From:" on the bottom of the label, USPS read it like they would an addressed envelope. I fought it (the extra charge to re-ship it) with the Post Office and won because they said it should have been caught by the postal employee when I dropped off the package for shipping at the Post Office.
