Only send it back if absolutley necessarry. Have had pretty good luck over the years.
Spent a year in Puerto Rico dining out just about every night. Talk about big cockroaches, they gottom.
Oh, generally go with the luck of the draw when it comes to servers.

I have had a steak or 2
that were undercooked before, but I won't return it - I'll eat what I can then bring the rest home & reheat it whenever and it comes out perfect. The reason I don't return anything is that my fallback job is waitressing (not by choice) and I have returned plenty of food & some cooks are a little temper-mental (mental-get it). I worked one place where the cook took the steak off the plate, threw it on the floor, stepped on it, then put it back on the grill, I was so embarrassed bring the steak back to the table, but what could I do....
I have also had people ask for me before, but I really didn't like it..

I typically only send a steak back if it is extremely over-cooked. I like mine rare-medium rare and if it comes out well done, I'll send it back - depends on the situation. If I get chicken and its undercook, well that's a no brainer!!
I worked in a restaurant for over 10years as a cook and it never bothered me when people sent food back.
No eggs for me - the only food that I do not like!!

I have sent undercooked meat back. A wonderful man and his wife opened a lovely restaurant not far from our home years ago. About 12 of us descended upon this new restaurant one Saturday evening after having made reservations. Now this man was a notable ex sports figure and many in our crowd followed his career and this was such a thrill to actually meet the gentleman.
I had ordered a steak, medium, twice baked potato and their famous Caesar salad. When my dinner finally came I found that the inside of the steak was frozen solid.  We got the attention of the owner and when I explained the problem, he just looked at me and said that couldn't possibly be. He then took the plate, cut the steak, or tried to,  and then announced, yes, it was indeed frozen.
I really felt sorry for him as this was only their 3-4th week in business. Anyway, he brought another plate with everything just perfect and later refused to take our money for my dinner. Of course we left a generous tip for the waitress. During this whole time she did everything possible to make our stay just right.
Sent back a steak at another restaurant for tasting really bad. Waitress asked if the steak had tasted as if it had sat out all night? I looked at her and said yes, it really tastes bad. She then admitted that the chef had inadvertently left the steaks to be cut on the cutting table all night!  . We left.

Yes, I send food back. I won't eat garbage, especially if I'm paying for it.
I almost sent back ribs at O'Charlies Saturday. Advertised as "fall off the bone meat" - it was "pull off the bone meat" (what there was of it) and "smothered in Chipotle sauce." The cook must have missed mine on the second sauce pass and he did a poor job on the first pass. Best thing on the plate was the rice. Two trips, two lousy meals. First time was some steak burger thing. Yuck! Mickey D's does better for a lot less money. Won't be going back to that restaurant. How do they think they'll stay in business serving lousy food? I don't get it. I will say my wife's chicken was delicious, and I don't much care for chicken.
We used to have a waitress at Chi-Chi's we asked for. In fact, when she would see us come in she would point at her area. I miss Chi-Chi's (before it was sold multiple times and totally ruined). We used to go every Saturday and meet 10-20 people in the bar. We would take over an entire section, get drunk and rowdy, and tip the waitresses very well for putting up with us. We heard they used to flip coins to see who would get to wait on us. My wife and I got to know the bar manager and his wife, the restaurant manager, and every time they came out with a new variety of margarita he would give us drinks and ask for our opinion. Inexpensive way to get polluted! They were really great people, and a large part of the reason it was so popular then. The building was torn down and replaced with a gas station/convenience store.

Sue,,,, you should have said something to the manager before you took those steaks back out to the customer! You are the go-between with the cook and the customer, I’d surely have told the cook that HE can take that plate back to the table and serve it after seeing what he had done. If I didn’t want to confront the cook or the manager, I’d have told the customer that they might think about going elsewhere, but I’d never return to the table after what a cook had done to the meal.

I mentioned that I'm not too picky about the food - Friends used to joke with me in the past about that. I'm sure most of you remember that Life Cereal commercial, where they show kids sitting at a table with Life Cereal in their bowls and they didn't know if they should eat it or not, and one of them said "Let's give it to Mikie", then another replies, "He won't eat it", then Mikie starts eating it, and the other 2 boys are surprised and they say "He likes it! Hey Mikie!" - Well, when I worked the graveyard shift around 1980 or so, we used to go to Coco's for breakfast in the morning after work, and a lot of times they'd joke with me about trying out food items on the menu! Of course, not being picky about what I ate, I always liked whatever I ordered and they'd do the same same thing they did in the Life Commercial, plus say some other things that couldn't be said on TV of course! 
Also, on food items being undercooked - when I order a dish with chicken in it, as long as the chicken is not flopping around, I'll eat it!

I've sent back food that wasn't cooked right or served cold. If I find a server I like we always ask for them.
