I like to give an age appropriate book, like Pat the Bunny, and a toy that reflects something in the book.
Or, like Scuffy, the tugboat, with a little boat.

Anything by Sandra Boynton or Eric Carlson. Very cute fun books for 1 yr olds.

An alphabet book, a book of colors, numbers, places, etc.

Get board books so he doesn't rip the pages out.

Melissa and Doug line of toys are also really good for little ones. Check for age appropriateness on the package if you're not sure. Target and Michaels have them.

You can't ever go wrong with sound effect books (almost, never anyway). For oldest grandson's first birthday, I gave him a book with farm animals sounds. He loved pressing the buttons, making the animals "moo", "neigh", "quack", etc. His aunt (my youngest daughter), also gave him a sound effects book that she found hillarious, called "Farts in the Wild", a humorous picture book depicting the exaggerated realistic sounds of jungle and domestic animals breaking wind. Her sister was less than thrilled, and the book "disappeared" for a few years on a closet shelf. Grandson (now almost four) has since rediscovered it, and it's one of his favorites.

Another first birthday gift he was given was a "Little People" Farm. It came with the basic barn and some animals, but there were additions that went with it, such as tractors, more animals, a pond, fence, etc., that he got for Christmas, Easter and his second birthday. He played with it for years and now shares it with his little brother.

Ditto this. My grandson has the "birthday cake" set and plays with it constantly. He has a favorite stuffed whale who has a birthday party every day.

Oh oh oh! As a mother of 4 young children, OP and all readers, I implore you, unless it's something the parents requested, please, please, please refrain from toys, especially books, that have a lot of sound effects unless you are willing to keep the items at your house for the child to play with when they come over!

Either I am just cheap, or know that one year old children are not much into anything...
with that said, the best gift is a cardboard box, wrapped or unwrapped. Voila - instant fort, boat, car, whatever a year old child can image.

I have pictures of my son on his first birthday: surrounded by toys and sitting inside the box one of those toys came in. That box was his favorite gift of all!
