If YOU paid the disability insurance premiums with after-tax income, the benefits are NOT taxable. If your employer paid the premiums without a deduction from your paycheck, the payments you receive are taxable.

Thank you guys. I have contacted the insurance company and was told that it is already included in my w2, so I am going to file on feb 16 without waiting for anything.
